Congratulations on being selected to take part in the National Health and Nutrition Examination Survey (NHANES). {When we contacted you recently, you were unsure about answering the survey questions.} If you decide to be in the study—and we hope you do—you will be helping to improve people’s health!

In the interview, we ask about your health and eating habits, among other questions. Your information will help doctors and researchers better understand — and improve — people’s health in the United States. That is why your participation is so important.


You answer only the questions you want to answer. Your answers are confidential. We do not share your information. You can decide to stop the interview or drop out of the study at any time.


As part of the study, you get a free health exam and the results to share with your doctor. We pay for your transportation to the exam and the costs of childcare or care of an adult if needed.


We will also give you {$MEC INCENTIVE} to thank you for taking part in the NHANES exam.

Why was I selected?

We want everyone to have an equal chance of being in the survey. We can’t go to every household in the U.S., so we randomly choose homes to visit. We want our information to reflect the many different types of people and homes in the U.S. You have the chance to represent thousands of other people in the U.S. like you!

How is NHANES useful?

For more than 60 years, NHANES has collected health and nutrition information. NHANES data has taught us a lot! For example:

  • It showed that high cholesterol causes heart disease.

  • We learned that a chemical caused babies to be born too small. Companies stopped using this chemical.

  • Even children’s growth charts used by doctors are based on what we have learned from NHANES.

NHANES data also help determine which health programs get funding and in which communities.

How long will this take?

Each interview takes about one hour. If you are busy, we can come back at a time that works for you.

Who runs this study?

NHANES is run by the National Center for Health Statistics at the Centers for Disease Control and Prevention.

How will my information be used?

We use your information only for health studies and reports. Your answers are added to others. No one can identify which answers are yours.

Assurance of Confidentiality – We take your privacy very seriously.  All information that relates to or describes identifiable characteristics of individuals, a practice, or an establishment will be used only for statistical purposes.  NCHS staff, contractors, and agents will not disclose or release responses in identifiable form without the consent of the individual or establishment in accordance with section 308(d) of the Public Health Service Act (42 U.S.C. 242m) and the Confidential Information Protection and Statistical Efficiency Act of 2018 (Title III of the Foundations for Evidence-Based Policymaking Act of 2018 (Pub. L. No. 115-435, 132 Stat. 5529 § 302)).  In accordance with CIPSEA, every NCHS employee, contractor, and agent has taken an oath and is subject to a jail term of up to five years, a fine of up to $250,000, or both if he or she willfully discloses ANY identifiable information about you.
