Registration is NOW open for data submission to the Agency for Healthcare Research and Quality's (AHRQ) Hospital Survey on Patient Safety Culture Comparative Database. Hospitals and health systems in the United States that have already completed administration of the AHRQ patient safety culture survey and are prepared to submit their data by JUNE 15, 2013 are eligible.

The AHRQ Hospital Survey on Patient Safety Culture 2012 Comparative Database Report was released on the AHRQ web site at http://www.ahrq.gov/legacy/qual/patientsafetyculture/hospsurvindex.htm. The 2012 report is an update of the 2011 report and includes data from 1,128 hospitals and 567,703 hospital staff respondents.
In addition, the 2012 report includes a chapter on trending that presents results showing change over time for 650 hospitals that administered the survey and submitted data more than once. The report consists of a narrative description of the findings and four appendixes presenting results by hospital characteristics (bed size, teaching status, ownership, and geographic region) and respondent characteristics (work area/unit, staff position, interaction with patients). A separate appendix on trending results by hospital and respondent characteristics is also presented.

8) Who you administered the survey to (All staff, a sample of staff, etc)
9) Survey Denominator (Number of staff the survey was administered to)
10) Survey Mode (Paper, Web, or Mixed Mode)
11) Month of data collection completion
12) Year of data collection completion
13) Survey questionnaire
14) Data file for each hospital
15) Information about Patient Safety Improvement Initiatives implemented between previous and most recent survey administrations for trending hospitals
