CMS will submit this package for a non-substantive revision within the next 6 months. The revisions will show that a note has been added to the information collection to indicate that the collection of demographic data is only for providers participating in Medicare. Specifically, the note shall read âNote: The reporting of race and ethnicity data are for Medicare providers only and is optional.â
CMS will also update the package to reflect the percent of data collected thus far that is specific to Medicare providers and will discard non-Medicare provider information as OMH is not authorized to collect such information.
CMS also agrees to the following conditions:
1) If the all-time response rate is 60 percent or more, then no conditions apply to the dissemination of the results.
2) If the all-time response rate is less than 60 percent, then CMS will not disseminate the results.*
*In subsequent years, if condition #2 above is met for three (3) consecutive years, then the CMS will discontinue the collection and explore alternate approaches for collecting providersâ race and ethnicity information.

The National Provider Identifier Application and Update Form is used by health care providers to apply for NPIs and furnish updates to the information they supplied on their initial applications. The form is also used to deactivate their NPIs if necessary. The original application form was approved in February 2005 and has been in use since May 23, 2005. The form is available on paper or can be completed via a web-based process. Health care providers can mail a paper application, complete the application via the web-based process via the National Plan and Provider Enumeration System (NPPES), or have a trusted organization submit the application on their behalf via the Electronic File Interchange (EFI) process. The Enumerator uses the NPPES to process the application and generate the NPI. NPPES is the Medicare contractor tasked with issuing NPIs, and maintaining and storing NPI data. The National Provider Identifier (NPI) Application processes over 1 million new provider and/or updates to existing application annually.
