The following will be presented to respondents at the beginning of the survey.

Thank you for participating in this survey. We are assessing the performance of a national test of the Wireless Emergency Alert system – a system used by the Federal Emergency Management Agency (FEMA) and State and Local authorities to send emergency alerts to cell phones.

A test alert was sent by FEMA on [DAY, DATE] to all cell phones in the United States. This survey will ask you whether you received that test alert, when you received it, and because we want to assess performance across the U.S., generally where you were at the time of the test (state, county, and city). We will also ask about other factors that may affect performance, such as your cell phone and carrier.

The survey is being conducted by the RAND Corporation, through its Homeland Security Operations and Analysis Center, on behalf of FEMA. The survey will take about 5 minutes to complete. Your identity will not be known to the research team or to FEMA, and your responses to the survey will not be connected to your identity. We do not anticipate any risk from participation.

Your responses are important to us, and your participation is voluntary. You do not have to answer any question if you are uncomfortable doing so. You may discontinue the survey at any time without penalty.
