We are recruiting youth age 13-17 to participate in a new survey about tobacco and public health. Your child’s participation in this survey can help researchers at Johns Hopkins University understand tobacco use in adolescents and young adults. The survey should take approximately 20 minutes to complete.

Points:

If your child qualifies and completes this survey, you will earn 150 Points. It will take 1-2 weeks

for your points to be credited to your account, and until then your Survey Status may

indicate "Started".
