Acceptable Proofs of Age
Are original proof of age documents required?
Generally, copies are acceptable. However, PBGC may request the original or a
certified copy of any proof document.
What document(s) are acceptable as proofs of age?
PBGC accepts a copy of any one of the following documents that provide the
individual's date of birth:
• Birth certificate
• Baptismal certificate or other church record
• U.S. Passport, expired or current
• U.S. Passport Card, expired or current
• Permanent Residence Card (Green Card)
• U.S. Certificate of Naturalization
Is a driver's license an acceptable proof of age?
No, PBGC does not accept a driver's license as proof of age.
What alternate proofs of age are acceptable?
If one of the above is not readily available, PBGC will accept any two from the list
below:

A notarized statement by the doctor or midwife who was present at the birth
indicating the date of birth as shown in the medical records.
A statement from the local office of the Social Security Administration showing
the amount of the Social Security benefit being received and the date of birth of
the recipient. A Social Security card is not acceptable proof.
A military record of service.
A marriage certificate, certified as correct by the custodian of the record.
A vaccination record.
A school record, certified as correct by the custodian of the record.

A copy of a birth certificate of the participant's child showing the participant's age
at the time of birth.
Adoption papers.
An accepted application for life insurance. This must be sent to PBGC by the
insurance company.
An employment record, certified as correct by the custodian of the record.
A labor union card, certified as correct by the custodian of the record.
Records of any fraternal organization to which the participant belongs, certified
as correct by the custodian of the record.
An official record of a foreign government or foreign church, certified as correct
by the custodian of the record.
A census transcript showing the participant's age. The participant may obtain this
document from the US Census Bureau by requesting a census search using a
US Census Bureau Form BC600.
A petition for Naturalization or Certificate of Citizenship.
An immigration visa or application for Permanent Residence.
Foreign passport, expired or current.

Must each document include the individual's date of birth?
One of the two documents must provide a date of birth. It is preferable that both
documents provide date of birth, but if that is not available, one alternate document may
specify "age" that validates the date of birth provided in another alternate document.
