NORC is conducting this survey on behalf of the Centers for Disease Control and Prevention (CDC), to assess the benefits and outcomes of the national public health accreditation program. The original email inviting you to participate was sent on DATE (below). Your participation will help us to better understand your overall experience, perceptions of the accreditation process, and the benefits, outcomes, and effects of accreditation.
