Local Government Resolution - Collateral Security Privacy Notice
GENERAL: This information is being collected for the primary purpose(s) of certifying local governments’
pledge of collateral for requested loans; documenting local governments’ agreement to pursue a loan;
and documenting local officials who have certified the approval of the applicable loan resolution.
AUTHORITY: Section 416 of the Robert T. Stafford Disaster Relief and Emergency Assistance Act, as
amended (42 U.S.C. § 5183) and 44 Code of Federal Regulations Sections 206.360-367
USES AND SHARING: Local Government Resolution - Collateral Security Forms are used by local
governments to comply with important Federal requirements, to pledge collateral for a loan, and certify
that the governing body of the local government has agreed to become indebted. FEMA may share the
information collected through these forms with the FEMA Finance Center in order to establish the
applicant’s loan account.
EFFECTS OF NONDISCLOSURE: The disclosure of information on this form is voluntary; however, failure
to provide the information requested may delay or prevent FEMA from providing the requested funding.
