MDL Collaboration Survey Instrument
Overview: This survey instrument is intended to be administered to the MDL project team members and educators. The goal of this survey is to assess the process of collaboration and co-creation which is an essential element for the design and implementation of this pilot platform and resource creation.
Instrument background: This document contains the draft set of survey questions for the Collaboration Survey. This survey derived from elements with Lynn Dierking’s Collaboration: Critical Criteria for Success and the Wilder Collaboration Factor Inventory (Mattessich, Murray-Close, & Monsey, 2001).

Introduction:
Hello! Thank you for making the time for this survey. It serves as a tool to help the project team examine and reflect on understanding of roles, shared sense of goals, expectations for communication and more. Your honest answers will help us improve how the team is working together. Your answers will remain confidential, and will only be used for the purposes of this study. No individual will be directly or indirectly identified, and all results will be presented in aggregate. The data collected for this survey will be destroyed upon the conclusion of the study.
