A species common or scientific name. One of Specific Species, General Species Type or Other Species, or Species Type is required. Specific Species A species type, such as Eagle, Hawk or Bat. Follow the link above for a list of accepted values. One of Specific Species, General Species Type, or Other Species or Species Type is required. General Species Type A species name or species type name. If the species name you entered in the Specific Species column is not found upon import, you may re-import your file with the species name here instead. However, first be sure to check for misspellings and other common names for the species. All known bat species should go here. Similarly, if a species type you entered in the General Species Type column is not found, you can enter it here. Number of Individuals Required. See the above link for accepted values. How Found Required. The date the animal was found. Date Discovered Optional: The date the animal or parts were collected from the field. Date Collected Either Latitude and Longitude, or County and State must be provided. Latitude Either Latitude and Longitude, or County and State must be provided. Longitude Either Latitude and Longitude, or County and State must be provided. The County entry must be just the county name. E.g., "Larimer", not "Larimer County". County Either Latitude and Longitude, or County and State must be provided. State The accuracy of the specified incident location. See the link above for valid values. Location Accuracy Optional. If you specified "Other" under Location Accuracy, use this field to describe the location accuracy. Other Location Accuracy The animal's condition. Alive or Dead. Condition Optional. If alive, indicate if the animal is injured or sick. If dead, indicate if carcass is intact, freshly killed [eyes moist], semi-fresh [stiff], partially decomposed, feathers and/or bones, etc. If you specified "Other" under Condition, indicate the condition here. Condition Details Required: Your best guess at the cause of mortality or injury. Follow the link above for a list of valid values. Suspected Cause Optional. Additional suspected cause details, such as what led you to conclude the suspected cause. If you entered "Other" under Suspected Cause, enter the cause here. Suspected Cause Details Required. Whether someone saw the injury or mortality occur. Yes or No. Event Observed Required. Where the animal ended up after its discovery. Follow the link above for a list of valid values. Disposition Optional. If the Disposition is "Other", describe the disposition here. Other Disposition Optional. If the Disposition is "Rehab Center", enter the rehab center name here. Rehab Center Name Optional. If the Disposition is "Rehab Center", enter the USFWS permit number of the rehabilitation center and/or the name and phone number of a contact at the center. Rehab Center Info Optional. If the Disposition is "Transferred to Other Permittee", enter the the USFWS permit number, and/or the name and phone number, of the other permittee. Permittee Info Optional. If the Disposition is "Sent to National Eagle Repository", enter the date sent. Date Sent to National Eagle Repository Optional. If the Disposition is "Lab", enter the lab name here. Lab Name Optional. If the Disposition is "Law Enforcement", enter the name and agency of the law enforcement individual that recovered the specimen. Law Enforcement Info Optional. The age of the animal. The five digit number on the FWS toe tag, if one was found on the animal FWS Toe Tag ID Optional. The unique identifier for this record in the source database. Source Record ID Optional.
