This year, your math intervention teacher is participating in a study. In this study, we want to learn if providing teachers training and support in a course can help them better teach math.

What will I have to do for the study?

During the school year, you will complete two math tests during your math intervention class—one at the beginning, and one at the end of the year. You will also take two questionnaires about your feeling towards math at the beginning and the end of the year. We would like to look at your answers to the math tests and questionnaires to help us better understand whether the course works.

What will happen to my test answers, scores or my answers to the questionnaires?

We will keep your answers and scores safe. We will not mention your name in any reports, or tell your test scores or your answers to anyone outside the research team. Your answers to the tests or questionnaires will not affect your school grades in anyway.

What if I want to say “No”?

Your parents have given permission for you to be a part of the study.

Yet, no one will be upset if you decide not to be part of the study. If you say no, your grades and school status will not be affected. You can also change your mind later. If you choose not to be in the study, you will still do all your normal intervention activities.

If you decide not to be in the study, please let your teacher know.

According to the Paperwork Reduction Act of 1995, no persons are required to respond to a collection of information unless such collection displays a valid OMB control number. The valid OMB control number for this information collection is 1850-xxxx. Public reporting burden for this collection of information is estimated to average 50 minutes per response, including time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. The obligation to respond to this collection is voluntary.
