This form is to be used in applying for authority to conduct program tests.
There must be a license application on file for the station at issue prior to filing this form.
Before filling out this application, the applicant should familiarize themself with the Communications Act of 1934, as amended, Parts 1, 2, and 73 of the Commission's Rules and Regulations, and the International and Telecommunication Union's Radio Regulations, as appropriate.
