Survey Introduction Email
[To be sent to all past HAMQAP participants]
Dear (HAMQAP participant),
NIST and the NIH Office of Dietary Supplements (ODS) are in the process of reviewing the HAMQAP program and are seeking feedback from all participants of the HAMQAP exercises to date. The feedback received from participants will be used to inform what, if any, changes are needed to make the program more valuable.
All participants’ feedback will be collected using a very brief electronic survey. A consultant will collect the information and will collate it without sharing specific responses to NIST or NIH ODS staff. Your responses will remain anonymous and will be combined with all other participants’ responses for reporting purposes.
Please click on the following link to begin the survey. We greatly value your experiences with the HAMQAP program and appreciate the time you take to help us find ways to improve the program. The survey will take approximately 20 minutes to complete and will remain open until XXX.
