The forms attached illustrate the changes we would like to make in the “Other” section of the Individual Application on pages 16 and 17. No other forms are affected by this correction. These edits
are to make the "Other" section consistent with the questions in the other misrepresentation sections and are based on feedback received from advocacy groups during the 30 day notice and
comment period. These changes were already made in six sections of the Individual Application but were overlooked in the corresponding text in the “Other” section when the forms were
submitted during the full information clearance process which was approved June 30, 2023.
