Instructions: This form is for International Military Students (IMS) only. Each adult Accompanying Family Member (AFM) must fill out the companion AFM form (DDF 3131). The Department of Defense (DoD)
utilizes the IMS's visa application (Department of State form DS-160) for most of the required biographic information. This form collects the biographic information required by DoD but not covered by the DS-160. If
the DS-160 has not been submitted, please fill in the supplemental page. This form has a front and a back. IMS from FVEY nations with U.S.-accepted security clearances do not need to complete this form.
