Why am I getting this letter?
Enclosed is your 2023 Hawaii Agriculture Disaster survey.
Please complete it at your earliest convenience. Your
information is important to measure the economic and
physical damage caused by the August 2023 wildfires and
high winds. Data collected from this survey will provide
the first indication of impact these events had on Hawaii
agriculture.

Online at agcounts.usda.gov. Online
reporting is fast and secure. All you
need is your unique survey code on the
enclosed form to begin.
By mail or fax. Complete the
questionnaire and mail it back in the
prepaid return envelope provided or fax
it to (855) 270-2722.

How will the data be used?
The Hawaii Department of Agriculture, other USDA
agencies, and organization supporting Hawaii farmers will
use the data to ascertain the extent of damage from these
events on crop and livestock production and to guide relief
efforts.
If we do not receive your completed questionnaire by Wednesday, November 15, 2023, we may attempt to
contact you to arrange a telephone interview or email you a reminder.
The information you provide will be used for statistical purposes only. In accordance with federal law, your
responses will be kept confidential and will not be disclosed in identifiable form. We will publish the data in
special report on Friday, January 19, 2024.
