Instrument to Request Inclusion on the List of US Exporters that FDA will Provide to China

To be included on the list of U.S. exporters that FDA will provide to China, manufacturers should email FDA at [email protected] and include all of the following information:

• Email subject line: (Your Firm’s Name): Facility Data for Export to China

• Permission statement (such as): (Your Firm’s Name) grants permission to FDA to share with the government of China’s GACC, the name and address (street, city, state, zip code) of the registered animal food facilities I have designated below.

• Individual facility data: For each registered facility you wish to include on the list, provide the facility name, street address, city, state, zip code. (Note: Information provided must match the data in FDA’s FURLS Registration database.)
