Healthy Start Program Survey Recruitment Email Invitation


Dear [PROJECT DIRECTOR’S NAME],


Thank you for your participation in the Health Resources and Services Administration’s (HRSA) Maternal and Child Health Bureau (MCHB) Healthy Start program. As you may know, HRSA is conducting a national evaluation of the Healthy Start program and has hired Westat, an independent research firm, to conduct this evaluation.


As part of HRSA’s evaluation, we are inviting you to participate in the Healthy Start Program Survey. The Program Survey is designed to collect information on Healthy Start programs’ infrastructure, activities, participants, community partnerships, new initiatives, and health equity. The web survey should take approximately 30 minutes to complete after you have consulted with your staff, and retrieved information from your Healthy Start systems and records. Please be sure to save your responses if you need to pause the survey for longer than 20 minutes. For more information about the survey, please refer to the Frequently Asked Questions (FAQs) attached. In addition, a copy of the survey is attached to show what questions will be asked. Please review this survey before going online to complete it.
