Healthy Start Participant Survey
HRSA’s Healthy Start Evaluation and Capacity Building Support Project
Dear Healthy Start participant,
We are inviting you and other participants to fill out a survey about the Healthy Start program. Your answers to the survey will help us improve the program. We want the program to do a good job meeting your needs.
The survey is voluntary. You may skip questions or stop the survey at any time. There are no right or wrong answers. Whether you do the survey or not won’t affect your Healthy Start services. Survey questions ask about your experiences with Healthy Start activities, health services, and health equity, which means that everyone has a fair and just opportunity to be as healthy as possible. Your answers are private. All answers are analyzed together, without any information that would identify a person who filled out the survey.
