Sealant Efficiency Assessment for Locals and States (SEALS)

This ICR data collection does not involve collecting information in identifiable form. The purpose of

this data collection is to assist states and communities in evaluating the effectiveness and efficiency of their
schools' dental sealant programs using an online database called SEALS. Access to the SEALS system is
password protected, and each local program and state administrator has a unique user name and password.
Only authorized CDC staff has access to SEALS as a CDC administrator.
Local school sealant programs can input aggregate child data or detailed child-level data. If states opt to use
detailed child-level data for programs in their state, then local programs must assign each child a unique ID #
that cannot be used by anyone outside of their program to determine the child's identity. Local programs are
granted access only to their program data, and cannot see data for other programs or states. State oral health
programs are also only able to view data from their state's local programs. It is the responsibility of the local
programs and states to maintain and protect data. CDC does not receive any identifiable data in SEALS.
The NCCDPHP Information Systems Security Officer has reviewed this submission and determined the Privacy
Act does not apply.
