Greetings Public Health Law Fellowship Alumnus,

As a graduate of the Public Health Law Fellowship, we request that you complete a brief survey. The survey asks a series of questions about your employment status, your job search, and your future career plans.


This is the third and final survey that we ask you to complete. Your responses will help us understand the career progression of graduated fellows and how the fellowship may or may not have contributed to that progression.


The survey is available in an electronic format: LINK. To access the survey, you must log in using the email address and password that you set up for you Secure Access Management System (SAMS) account.


Please follow the instructions on the screen to complete the survey online.


The survey is estimated to take 10 minutes or less to complete. Your open and honest feedback is appreciated. Your participation is voluntary, and you may skip questions or stop the survey at any time without any adverse consequences. Findings will be included in a report to CDC that may be publicly available. In this report, data or quotations will not be linked to the identity of a particular respondent or organization.
