Greetings Public Health Law Fellowship Host Site Supervisors,

As your fellow’s time as a public health law fellow ends, we request that you complete a brief survey. The survey asks a series of questions about the projects your fellow worked on or completed, your perceptions of public health law, policy, and related topics, and about your experience as a host.


This survey is the second of two surveys we will ask you to complete. Your responses will help us understand the needs of the public health law workforce and evaluate how the Public Health Law Fellowship does or does not meet those needs.


The survey is available in an electronic format: LINK. To access the survey, you must log in using the email address and password that you set up for you Secure Access Management System (SAMS) account.


Please follow the instructions on the screen to complete the survey online.


The survey is estimated to take 12 minutes or less to complete. Your open and honest feedback is appreciated. Your participation is voluntary, and you may skip questions or stop the survey at any time without any adverse consequences. Findings will be included in a report to CDC that may be publicly available. In this report, data or quotations will not be linked to the identity of a particular respondent or organization.
