Greetings Public Health Law Fellow,

We look forward to working with you all soon. In advance of your start date, we request that you complete a brief survey before the start of the fellowship. This survey asks a series of questions about your current knowledge of various public health law topics and expectations for the fellowship. Your responses will be used only to help us understand baseline knowledge of incoming fellows and if reported will not be directly linked to any survey respondent.


This survey is the first of three that we will ask you to take, with the second occurring at the close of your fellowship and the third occurring six months after your last day in the fellowship or after you graduate, whichever is later. Your responses will help us assess whether and how the fellowship affects your professional skills and career progression. Additionally, your responses will help us evaluate the fellowship program itself.


This survey asks for your race, gender identity, disability status, and a series of questions about public health law. We ask for this information to help us ensure an equitable experience for all participants and to assess the level of diversity in our fellowship program. These demographic data will be reported only in the aggregate. You may also select "prefer not to respond" to questions about your race, gender identity, and disability status.


The survey is available in an electronic format: LINK. To access the survey, you must log in using the email address and password that you set up for you Secure Access Management System (SAMS) account.


Please follow the instructions on the screen to complete the survey online.


The survey is estimated to take 6 minutes or less to complete. Your open and honest feedback is appreciated. Your participation is voluntary, and you may skip questions or stop the survey at any time without any adverse consequences. Findings will be included in a report to CDC that may be publicly available. In this report, data or quotations will not be linked to the identity of a particular respondent or organization.
