AHRQ Primary Care Learning Community Participant Survey





Thank you for being part of AHRQ’s Primary Care Learning Community over the past year. We convened the Learning Community with the goal of supporting primary care practices during the COVID-19 public health emergency. Over the past year, we invited partners from national, regional, state, and local organizations who work directly with primary care practices to share experiences and resources to address the new and ongoing needs of primary care during this public health emergency.

We are inviting you to complete a brief survey to understand what worked and what we can improve for future Learning Community activities. The survey will take about 5 minutes to complete. Your participation is voluntary and you may skip any questions you do not wish to answer. Please be assured that your data will be stored in a secure manner and kept strictly confidential. Your responses will only be used internally by AHRQ to help improve future learning community activities and will not be publicly released.
