The study is being sponsored by the U.S. Department of Agriculture (USDA), Food and Nutrition Service, the Federal agency that oversees the SNAP and SNAP E&T program and others like it around the country.

Who is conducting the study?

The study is being conducted by Mathematica, an independent research firm. We do not work for the SNAP [NAME OF STATE PROGRAM] agency, USDA, or any other governmental agency.

Why should I participate?

Is the information kept private? How is the information going to be used?

The information you provide will be considered private. We will summarize the information we collect and put it together with information that we gather from others. Nothing you say will ever be linked to your name. All information will be stored safely and destroyed at the end of the study. Also, nothing you say will affect your eligibility for SNAP, SNAP E&T or any other programs.

Do I have to participate?

No. Your participation today is voluntary. You don’t have to answer any questions you don’t want to, and you can leave at any time.
