In-Depth Interviews

Participation Consent Form


You have been invited to join our study about food safety and take part in an online interview session. This form describes the purpose, process, benefits, and risks of taking part. It also describes your right to stop the interview at any time. If you wish, a member of the study staff is available to read through this form with you and discuss any of the information.


Who is the sponsor of this study?

The U.S. Department of Agriculture (USDA) Food Safety and Inspection Service (FSIS) funds the research. The study is created by their Office of Public Affairs and Consumer Education (OPACE).


Fors Marsh is helping conduct this project on behalf of the USDA.


What is the research for?

To get input on attitudes, feelings, and behaviors related to food safety to help inform a new USDA FSIS Food Safety campaign.


Who is taking part?

We aim to have thirty total study participants involved in either English or Spanish-speaking interview sessions.


What must I do before the session?

You will not have homework. Be sure to receive an email with instructions for joining the online session.


What are the risks of joining the study?

The risks are minimal. You may feel embarrassed or unsure about sharing your responses to some questions.


How might I benefit from taking part in the study?

You will get no direct benefits. But the findings of this study will be used to help families and society at-large regarding food safety.


What will happen during the session?

The session will be in [English/Spanish] and last about 60 minutes. You will be asked questions about:

  • Your feelings about and behaviors related to food safety.

  • How your culture impacts your food safety behaviors.

  • How you prefer to learn about food safety.


Do I have to take part in the session?

You choose whether to join the session. After the session starts, you can choose not to answer any question you do not wish to answer. You can leave the session at any time. This will bring no penalty to you, and you will lose no benefits.


Will I get paid for joining the session?

You will get a payment worth $75 for your time. You will get it through email after completing the session.


What will the research team do with my input?

Someone on the research team will:

  1. Audio record the session.

  2. Listen and take notes during the session.

  3. Use the notes to write a report.


How will the research team keep my input private?

No one will be able to link what you said to your name or other personal information. The research team will:

  1. Keep the notes and recording of the session in a password-protected computer folder.

  2. Keep your personal information in a separate folder from your responses. All responses cannot be linked directly to your name.

  3. Summarize the data from everyone interviewed in a report. This report will not connect you with your answers.
