State Digital Equity Capacity Grant Program Digital Equity Plan Amendments Description Form
This form will serve as a tool to capture amendments made to the Digital Equity Plan of the State or Territory as part of the application to the State Digital Equity Capacity Grant Program (SDECGP). Applications for the SDECGP from States must be received no later 11:59 p.m. ET on [DATE], 60 days after the issuance the SDECGP NOFO. Applications from U.S. Territories must be received no later 11:59 p.m. ET on [DATE], 120 days after the issuance the SDECGP NOFO. Per Section II.C.2. of the SDECGP NOFO, any State or Territory applying for the SDECGP that has amended its Digital Equity Plan must submit the amendments with the application for the SDECGP. Any and all amendments the State or Territory made to the Digital Equity Plan originally accepted by NTIA must be documented in this form using the "Amendments" tab.
