RE: FDIC Chairman Event Request Form

Under its generic clearance entitled, “Information Collection for Qualitative Research” (3064-0198), the FDIC hereby submits for OMB review a questionnaire entitled “FDIC Chairman Event Request Form.” The purpose of the questionnaire is for external parties to invite the FDIC Chairman to participate in their event and for the FDIC to determine the purpose, logistics, credibility, and risk profile of the event. For these purposes, the questionnaire requests a description of the organization, event, attendees, logistical information, and a copy of the draft agenda or run-of-show. Additionally, the form requests information related to the level of participation by the Chairman, press access, and the identity of other invited speakers.


The questionnaire is made available through the FDIC’s public website (www.fdic.gov). Interested parties submit the simple application request online. The FDIC will use the application form to determine whether the purpose of the event aligns with the FDIC’s mission and the availability of the Chairman.

Annual Estimated Number of respondents: 60

Estimated Time per response: 7 minutes

Total Estimated Annual Burden: 7 hours
