A few weeks ago, a parent or caregiver in your household responded to the National Survey of
Children’s Health – Longitudinal Cohort (NSCH-LC) online. We sincerely appreciate your
household’s time and willingness to complete the first part of the survey. Based on the information
your household already provided on one child or young adult, we would now like to ask you important
survey questions about a second child or young adult that you listed. The questions we would like to
ask about this second child or young adult are included in the enclosed booklet. We hope you will

What is the National Survey of Children’s Health—Longitudinal Cohort (NSCH-LC)?
The NSCH-LC is a national survey that focuses on the physical and emotional health of children
and young adults ages 3 to 24 in the United States. The 2023 NSCH-LC collects information
related to the health and well-being of children and young adults, including access to and use of
health care, family interactions, mental health, school and after-school experiences, and family
economic circumstances.
Why is this survey important?
The NSCH-LC is designed to measure the impacts of the COVID-19 pandemic on children, young
adults, and their families. The results of the NSCH-LC will help inform the development of federal
policies and programs aimed at improving the health and well-being of children and young adults.

Why should I participate?
Your household is unique and important. We need to hear from households from all walks of life
to collect enough information to help improve the health of children and young adults across the
country. If a household that has been selected for the survey does not respond, we cannot
replace them with another household.
I consider this child an adult. Do I still need to answer the survey?
Yes. The NSCH-LC asks questions about both children and young adults ages 3 to 24. The
survey also asks questions about their families and the experiences of young people as they
transition into adulthood.
This child or young adult that does not live with me. Do I still need to answer the survey?
Yes, even if the child or young adult does not live with you, we still need you to respond.
How long will this survey take?
For most households, this final part of the survey should take, on average, 35 minutes to
complete.
Will my response be confidential?
The U.S. Census Bureau is required by law to protect your information. We are not permitted to
publicly release your responses in a way that could identify your household. The Census Bureau
is conducting this survey under the authority of Title 13, United States Code (U.S.C.), Section 8(b)
(13 U.S.C. § 8(b)) and Section 501(a)(2) of the Social Security Act (42 U.S.C. § 701). Federal law
protects your privacy and keeps your answers confidential under Title 13, U.S.C., Section 9 (13
U.S.C. § 9). Per the Federal Cybersecurity Enhancement Act of 2015, your data are protected
from cybersecurity risks through screening of the systems that transmit your data. This survey has
been approved by the Office of Management and Budget (OMB). The eight-digit OMB approval
number is 0607-1028.
