practices, the nutritional quality of school meals and snacks, the cost to produce school meals, and student participation
and dietary intakes. This is a voluntary collection and FNS will use the information to monitor program operations. This
collection does not request any personally identifiable information under the Privacy Act of 1974. According to the
Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to, a
collection of information unless it displays a valid OMB control number. The valid OMB control number for this

[SCHOOL NAME] is part of the 2024 – 2025 National School Foods Study. This
important study for the U.S. Department of Agriculture (USDA) Food and Nutrition
Service (FNS) is being conducted by Mathematica, a research firm that has conducted
many studies related to school meals. FNS is sponsoring this study to learn about the
nutritional needs of students across the country and your satisfaction with school meals.
Your school district was selected to participate and supports this research.
Your child has been selected for the study. Mathematica interviewers will meet with
students who eat school meals and those who do not eat them. They will visit your
school in the next few weeks to meet with students and staff. [G2a: The study team will
also contact you for a parent/guardian survey. G2A: If you choose to participate, you will
receive a (ELEMENTARY: $25; MIDDLE/HIGH: $15) gift card to thank you for your help.]
Your child will receive a (ELEMENTARY: $5; MIDDLE/HIGH: $15) gift card for taking part
in the study. Study participation includes these two steps:

“WHAT I ATE” SURVEY
[ELEMENTARY G2A] Students, with help from their parents/guardians, will report what
they ate and drank on one particular day. The “What I Ate” survey has two parts. Part 1
will be completed with your child at school on the selected day. Part 2 will be completed
with you and your child together. Part 2 can be in-person at school with your help on the
following day, or by telephone within 48 hours. We can schedule this at a convenient
time for you. Some families may be asked to complete the survey a second time a few
days later by telephone. These families will receive an extra $25 via gift card. More
information about this second survey will be provided during the first survey.
[ELEMENTARY G2B] Students will report what they ate and drank in school on one
particular day. The “What I Ate” survey will be completed with your child at school on
the selected day.
[MIDDLE/HIGH G2A] Students will report what they ate and drank on one particular day.
The “What I Ate” survey will be completed with your child at school on the selected day.
Some students may be asked to complete the survey a second time a

few days later on the phone. These students will receive an extra $15. More information
about this second survey will be provided during the first survey.

STUDENT [G2A: AND PARENT/GUARDIAN] INTERVIEWS
Our interviewers will hold a short interview with your child after finishing (ELEMENTARY
G2A: the student part of) the “What I Ate” survey. They will ask your child’s opinion of
school meals and snacks and reasons for choosing or not choosing them. Your child will
receive a (ELEMENTARY: $5; MIDDLE/HIGH: $15) gift card after the in-person interview.
[G2A: Our study team will also interview you for your opinion on school meals. You
may complete the interview either in-person, on the web, or by telephone.]
Taking part in the study is voluntary. That means you can choose to participate or not and
your child can choose to participate or not. Being in the study or choosing not to be in the
study will not affect any benefits you may receive. You or your child can skip any
questions you do not wish to answer and can stop being in the study anytime.
The “What I Ate” survey and interview will take about [G2A: 60; G2B: 30] minutes of

The information collected will not be seen by anyone at your child’s school or the school
district. It will only be used by researchers for statistical purposes. Neither you nor your
