This checklist helps entities through the Phase 3 application by providing a brief overview of each
application component.

Readiness Assessment
•

Complete QECP Letter of Commitment (template provided by QECP Team,
and located in the QECP Phase 3 Toolkit) demonstrating compliance

•

Provide rationale for selecting the Standard Measure, information on the
relationship to existing measurement efforts, and relevance to the
population examined

•

Attest (yes or no) whether the QE is following the Standard Measure
specifications and guidance for risk adjustments, outliers, and attribution

•

Complete QECP Measure Information Workbook (template provided by QECP
Team, also located in the QECP Phase 3 Toolkit) demonstrating compliance
with the program requirements for any Alternative Measures selected for
public reporting

•

Provide evidence that proposed Alternative Measures are more valid,
reliable, responsive to consumer preferences, cost effective, or relevant to
dimensions of quality and resource use measures not addressed by Standard
Measures

•

Provide documentation of stakeholder engagement and support for the use
of Alternative Measures

•

Provide prototypes of the confidential provider performance report and
public performance report containing an understandable description of the
measure used to evaluate the performance of providers or any provider
rating approaches
