Cognitive Interviewing Consent Form

We would like to speak with you about a survey Westat is conducting for the U.S. Department of Labor. The survey is about where people work, the jobs they do, and how they are paid. By taking part in this interview, you are helping us evaluate how easy or difficult the questions are to understand and answer. Your opinions will help us improve the questionnaire.

• Your participation is voluntary. You can skip any question and you can stop at any point.

• The information you provide in this study will be treated as secure and will be only used to improve the survey. We will not share your information with anyone outside of the Westat research team.

• The interview will be conducted online using Zoom and will last about 90 minutes.

• We would very much appreciate your permission to audio and video record this session. The recordings will be used for note-taking purposes only and may be used only by the Westat research team. They will be destroyed as soon as we complete the study. You may still participate in the interview even if you do not want to be recorded.

• Your answers to the questions will not be linked with your name and we will not use your name in any reports. We may use quotes in our report that you provide during the interview, but never in a way that would identify you.

• We will email you a $50 electronic Amazon gift card in appreciation for your time and assistance.
