Nonsubstantive Change Request

In late 2022, APHIS received a petition from the Animal Legal Defense Fund and Compassion Works International, challenging us to hold our licensees and registrants more accountable for accurately reporting deaths of animals in their possession.  They noted that the current versions of these 6 forms are not formatted in such a way as to ensure that all deaths are accurately captured.  We concurred and have revised the forms accordingly.  We are submitting these for approval as nonsubstantive changes, as no new information is being requested under the regulations and burden estimates are unaffected.

• Corrected burden estimate in OMB banner

• Minor revisions to headers in Columns A, E, and K.

• Added Column L, “Cause of Death or Reason for Euthanasia”
