  1. Does the dog have a current and valid Certification of U.S.-Issued Rabies Vaccination form?

___ If Yes, the dog(s) can be cleared for entry.

  1. Does the importer have a reservation at a CDC-registered Animal Care Facility [enter name of ACF]?

___If Yes, the dog will be placed on hold by CBP, and require a 1F transfer to the CDC-registered Animal Care Facility. The facility will notify CBP and CDC when entry requirements have been met. CDC will provide the final concurrence for entry.
