Does the importer have a reservation at a CDC-registered Animal Care Facility [enter name of ACF]?
___If Yes, the dog will be placed on hold by CBP, and the airline should post-enter the dog(s) and transfer the dog(s) to cargo. Once in cargo, a 1F transfer is required to move the dog/s to the CDC-registered Animal Care Facility. The CDC-registered Animal Care Facility will notify CBP and CDC when entry requirements have been met. CDC will provide the final concurrence for entry.
