Justification for Non-Substantive Changes to the Collection or Resubmission of the Collection within One Year of OMB Approval

We are making non substantive changes to form SSA-1372-BK within One Year of OMB approval to meet the agency’s accessibility standards.

• Change #1: We are increasing the font sizes; reformatting page 2 so that it takes up 2 pages instead of 1; and increasing the number of pages for this form. When a vendor prints the SSA-1372-BK the form must of 12 pages instead of the current 8, as forms like this print in groups of 4. This change requires adding blank pages, which allows pages to be removed as needed and send to different people or places. The 12-page layout is only for the vendor-printed version. The version of the SSAm-1372-BK posted on the Social Security Administration’s Intranet and www.ssa.gov will remain in the 8-page layout. This allows individuals to print the form on their own printers and keep or send each page as needed.

Justification #1: We are making these changes to meet the agency’s accessibility standards for low vision users . Currently, much of the content on page 2 is in very small font that causes problems for those with vision issues.

• Change #2: We are revising the instruction language on pages 1, 4, and 5, to reflect the new page numbers.

Justification #2: We are revising the instruction language due making format changes on page 2.

This action does not affect the public reporting burden. SSA will implement the changes upon OMB approval.
