Public Reporting Burden Statement

HUD collects and uses this information to assist in the planning for a detailed follow-up evaluation of HUD’s Lead-Based Paint Hazard Control Grant Program (“National Evaluation”). This information is required to obtain benefits because participation in HUD's data collection activities regarding the Lead Hazard Control grant is required as a condition of that grant funding.  This information collected will not be held confidential.
