This information is being collected to assist the Food and Nutrition Service in understanding school food purchasing practices, the nutritional quality of school
meals and snacks, the cost to produce school meals, and student participation and dietary intakes. This is a voluntary collection and FNS will use the information
to monitor program operations. This collection does not request any personally identifiable information under the Privacy Act of 1974. According to the
Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it

information collection is estimated to average 0.05 hours per response, including the time for reviewing instructions, searching existing data sources, gathering

2024 – 2025 National School Foods Study
Student Assent
Who is conducting the study? The study is for the U.S. Department of Agriculture (USDA) Food
and Nutrition Service (FNS). Mathematica is a research company that was hired to collect all the
information for the study.
Why is this study being conducted? The USDA is responsible for the school meal programs
and they are conducting this study to learn what kids like you think about the school meals.
What do I have to do? A study interviewer visiting your school will ask you questions about
yourself, school meals and snacks, and what you ate. At the end, the interviewer will give you a
gift card for helping with the study.
Do I have to participate in the study? No. You can decide if you want to be in the study or not.
There are no known benefits to participating in this study. You can decide not to answer any
questions you don’t want to answer.
Will anyone know I am in the study? No. Your name will not be part of the study, and your
answers will be kept private.
Will my answers be kept private? Yes. All information gathered for the study is private under
the Privacy Act to the extent allowed by law. The person who will interview you is not allowed to
share your personal information with anyone except other people working on this study. Your
name will not be part of the study and will not be shared with the USDA. As required by law, we
will only share information if there is a risk of harm to yourself or others.
What if I do not eat school meals? Even if you do not eat the school breakfast, lunch, or snacks,
we still want you to be in the study. It is important for the study to learn about both students who
eat school foods and students who do not.
Who can I talk to about the study? If you or your parents have any questions about the study,

or your parents have any questions about your rights as a research participant, please call HML

It is your decision to participate in the study. Anything you say to the study interviewer will
only be shared with the people working on this study. You can tell the study interviewer at
any time if you decide not to participate. You are always free to change your mind.

¿Quién está conduciendo el estudio? El estudio es para el Departamento de Agricultura de los Estados
Unidos (USDA, por sus siglas en Inglés)/Servicio de Alimentos y Nutrición (FNS, por sus siglas en inglés).
Mathematica es una empresa de evaluaciones contratada para recoger toda la información para el estudio.
¿Por qué se realiza este estudio? El USDA es responsable de los programas de comidas escolares y está
realizando este estudio para saber qué piensan los niños como tú de las comidas escolares.
¿Qué tengo que hacer? Un entrevistador del estudio visitando tu escuela te hará preguntas acerca de ti, las
comidas y meriendas escolares, y lo que comiste. Al final, el entrevistador te dará una tarjeta de regalo por
ayudar con el estudio.
¿Tengo que participar en el estudio? No. Tú puedes decidir si quieres o no quieres estar en el estudio. No
se conocen riesgos ni beneficios por participar en este estudio. Puedes decidir no responder a las preguntas
que no quieras contestar.
¿Sabrá alguien que yo estoy en el estudio? No. Tu nombre no será parte del estudio, y tus respuestas se
