This information is being collected to assist the Food and Nutrition Service in understanding school food purchasing practices, the nutritional
quality of school meals and snacks, the cost to produce school meals, and student participation and dietary intakes. This is a voluntary collection
and FNS will use the information to monitor program operations. This collection does not request any personally identifiable information under the
Privacy Act of 1974. According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to
respond to, a collection of information unless it displays a valid OMB control number. The valid OMB control number for this information collection
is 0584-[xxxx]. The time required to complete this information collection is estimated to average 0.02 hours per response, including the time for
reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection

The 2024 – 2025 National School Foods Study will be visiting our school tomorrow. The study
is for the US Department of Agriculture Food and Nutrition Service to learn how to improve
school meals. If your parent agreed for you to participate, a member of the study team may ask
you to take part in the study. Information about the study was previously sent to your home.
Reminders for students are below. As a reminder, all study information will be kept private.
What do I have to do?
• The study team will ask you some questions about the foods that you usually eat
during the school day, including meals obtained at schools.
What do I get for participating?
• You will get [$5/$15] if you finish an interview in school. (There may not be time
for everyone to take part.)
[G2A only: What do parents have to do?
•

[ES only: The study team will call parents in the next few days from a 609 area code to ask
some questions. They will ask parents about what their child ate on the day they were
interviewed in school. Parents will receive a $25 gift card for taking part in the study.]

[ES only: Each child will bring home a food diary. Parents should fill out the food diary for
the child on the same day the child was interviewed in school. It will help during the
interview to remember what the child ate.]

[ES only: Each child will also bring home a food model booklet, measuring cups and
spoons, and a ruler. These are to help estimate the size of the food the child ate. You can
keep these.]

[MS/HS only: The study team will ask parents to complete an interview about school meals.
The parent interview can be completed by web or phone. Parents will receive $15 for taking
part in the survey.]]
