Subject: Notification of Usability Survey

Dear [NAME],

Thank you for your continued participation in the study on prostate cancer screening funded by the Centers for Disease Control and Prevention (CDC) Division of Cancer Prevention and Control. As a next step in the study process, we would like to invite you to complete a brief usability survey to gather information about your perceptions of Talk to Nathan About Prostate Cancer Screening that you recently reviewed. The primary purpose of this survey is to get your input on Nathan related to how well the information was presented; the length, pace, and amount of information presented; your satisfaction with Nathan; and how useful Nathan was for making decisions on prostate cancer screening.

Within the next week, you will receive an invitation to participate in the survey. The survey will take approximately 18 minutes to complete.
