Recently, you received an invitation to participate in the Post-Clinic Visit Survey for the study on prostate cancer screening funded by Centers for Disease Control and Prevention (CDC) Division of Cancer Prevention and Control. We are writing to you because you have not completed the survey. The survey will be open until [DATE], and we would greatly appreciate your participation. Your participation will help the study by providing us with valuable information about your visit with your primary care provider and how much the interaction with the provider helped you with your decision regarding prostate cancer screening.

The survey will take approximately 20 minutes to complete. Your personal identity will be kept confidential. The completed surveys will be kept secure and separate from any information that identifies you such as your name. If this study is published or presented at scientific meetings, names and other information that might identify you will not be used.
