Exhibit C: Request for Site-Level FNS-418 Data
We are asking each State to provide site-level administrative records data based on the data your State
submitted for the FNS-418, Report of the Summer Food Service Program for Children, for Federal fiscal
year (FY) 2024 (October 2023 through September 2024). Fulfilling this data request will help FNS gain a
deeper understanding of Child Nutrition Program operations in FY 2024. The information you provide is
particularly important during this time when FNS is assessing program operations as sites and sponsors
return to normal operations after COVID-19.
Please provide the FNS-418 Report of the Summer Food Service Program for Children at the site level for
FY 2023. Table 1 at the end of this document shows all the data elements we are requesting for each site.
Although some of these data are reported to FNS annually, we are requesting monthly data for each
section of the FNS-418 form.
Please provide the data in whatever format is easiest for you. For example, you could provide separate
files for each month or a single Excel file with separate tabs for each month.
In addition to the FNS-418 data, for each site please provide:

Data delivery timeline
Please provide the data file for FY 2024 by April 1, 2025.
The Food and Nutrition Service (FNS) is collecting this information to understand Child Nutrition Program operations in FY 2024 and will use the
information to inform planning, policy, and guidance related to the Child Nutrition Programs. Because the personally identifiable information (PII)
requested under this collection will not be used to retrieve survey records or data, requirements of the Privacy Act of 1974 do not apply. Responses
will be kept private to the extent provided by law and FNS regulations. According to the Paperwork Reduction Act of 1995, an agency may not conduct
or sponsor, and a person is not required to respond to, a collection of information unless it displays a valid OMB control number. The valid OMB
control number for this information collection is 0584-0607. The time required to complete this information collection is estimated to average 4.0 hours
per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and
completing and reviewing the collection of information. Send comments regarding this burden estimate or any other aspect of this collection of
information, including suggestions for reducing this burden, to: U.S. Department of Agriculture, Food and Nutrition Service, Office of Policy Support,

Data delivery and storage
Most data files requested for this study, including any files requested in the web survey, can be submitted
via email. However, if the data files contain sensitive information (for example, the names of individuals)
or if an extra level of security is preferred, secure options for transmitting the files include passwordprotecting the files, sending the files through a secure transfer platform used by the State agency, or
uploading them to Mathematica’s secure transfer site hosted by Box. We can accept data in any standard
format. We will store the data in a secure project directory where only those working with the data will
have access.
