This is a friendly reminder to complete the [EIS/LLS] Alumni Survey. We greatly appreciate your feedback to help us to continually improve our program. The survey should take approximately [Year 1: 25, Year 3/5: 20] minutes to complete.
Your privacy is important to us. Your individual responses will be kept secure and not shared. Only CDC Epidemiology and Laboratory Workforce Branch (including [EIS / LLS] program staff) and CDC Division evaluation staff will have access to your responses. Results from the survey will be reported in aggregate (e.g., 75% of alumni are employed in public health).
