This is a request for OMB to approve non-substantive changes to the attached CTSU OPEN Rave
Request Form. The updates to the CTSU OPEN Rave Request Form are as follows:

Clarification for questions 2.8, 3.4, and 4.7;
Addition of a list of values for the response in question 2.8;
Reduction of data collection in question 4.3; and
Removed questions 4.5 and 4.6.

The updates to questions 2.8, 4.3 and 4.7, and the removal of questions 4.5 and 4.6 will not increase
the burden to complete the form.
Attached to this request is the revised form (clean and tracked versions). All the changes can be
seen in bold red (additions and strikes) in the “Revised Marks” file.
