SUBJECT: ESRD QIP Data Validity and Reliability Study – Request for Medical Records
The Centers for Medicare & Medicaid Services (CMS) contracted with XXXXX to assess the accuracy of the Clinical Performance Measures (CPM) data entered into the CROWNWeb system. We will also validate positive blood culture “candidate dialysis events” to assess the accuracy of the Dialysis Event data entered into the Center for Disease Control (CDC) National Healthcare Safety Network (NHSN) system.
Facilities that will be randomly selected to participate in this effort should submit their patient medical records using the following options:

Among the findings of the Calendar Year 2017 study is that data for Access Type, Kt/V and Session Date failed some of the quality checks that were applied to CROWNWeb data. Consequently, the percent correct matching results were lower than expected in these instances.
RELI recommends to facilities that want to maximize their percent correct matching outcomes should:
Ensure that data for all required fields are entered in CROWNWeb; and
Check that entered data values are consistent with other data values (e.g., Session Date value is not before the Admit Date value or after the Discharge Date value).
