Notes on Updates
*Transitioned from Hospital Inpatient Quality Reporting (IQR) Program PRA
package to the HACRP PRA package in FY 2025. The OMB control number
has been updated accordingly.
• Removed references to PC-01 and the Hospital IQR Program.
• Replaced “Colon Surgery (SSI Colon and SSI Abdominal
Hysterectomy)” with “Colon and Abdominal Hysterectomy Surgical
Site Infection).”
• Re-organized language under “Specified CAUTI and CLABSI
Requirements” for brevity.
• Added language clarifying that the Measure Exception form must
be submitted each year of a performance period.
• Replaced use of “exempt” with “except.”
• Made minor grammatical and annual updates as needed (e.g., the
publishing date of the form).
