Justification for a Non-Substantive Change for
NOTICE OF ALLEGED SAFETY AND HEALTH HAZARDS (OSHA-7 Form)

During the submission to OMB, the agency inadvertently uploaded into ROCIS the wrong OMB Control Number and the wrong expiration date at the time of the request to OMB on the forms. OSHA is requesting an update to the OSHA 7 Complaint Forms to correct the error for both the English and Spanish versions.
Currently the forms read:
OMB Control Number: 1218-0262 – Expires: 5-31-24
Requesting the forms to read:
OMB Control Number: 1218-0064 – Expires: 8-31-2024
