Fed/State Supplemental Funding Final Report
NEH uses reports to evaluate the work of grantees, assess the impact of its funded projects across the country, and to explain the
value of this work to Congress and the American public. Please answer the questions on this tab, as they are all required. All fields
must be filled in order to save your work. Please review your responses before submitting.For councils that made subawards,

The Office of Management and Budget requires federal agencies to supply information on the time needed to complete forms and
to invite comments on the paperwork burden. NEH estimates that on average it takes 4-5 hours to complete the Final Performance
Report. This estimate includes time for reviewing instructions, researching, gathering, and maintaining the information needed, and
completing and reviewing the application. Time needed may vary from program to program. You may send any comments
regarding the estimated completion time or any other aspect of this report, including suggestions for reducing the completion time,
to NEH's Office of Federal/State Partnership, [email protected], 400 Seventh Street SW, Washington, D.C. 20506. According to the
Paperwork Reduction Act of 1995, no persons are required to respond to a collection of information unless it displays a valid OMB
number.
