The RTI International (RTI) and Implenomics Survey Teams are working on behalf of the U.S. Centers for Disease Control and Prevention (CDC) to collect information about barriers that newly diagnosed patients and their caregivers face at each stage of cancer care: screening, diagnosis, treatment, and post-treatment. We are also interested in learning about the barriers caregivers perceived their care recipients faced. We are inviting the adult caregivers of individuals who were first diagnosed in 2021 with colorectal, female breast, or cervical to participate in the study. We received your contact information from [NAME], who identified you as their caregiver. The survey contains questions about your personal and health background, your experience as a caregiver, your quality of life, and patient’s barriers to care from the caregiver’s perspective.
If you take part in the survey, there are protections in place to keep your responses confidential and private , and your name will not be linked to your responses. Your responses will be combined with other caregivers’ responses for analyses. We hope you choose to participate because your experiences and perspectives are valuable.
Completing this survey is voluntary and should take only about 15minutes. You will have up to 2 weeks to complete the survey. After you complete the survey, we will send you a $40 check or gift card to thank you for your participation. At the end of the survey, you can also volunteer for a 60-minute interview and 20 participants will be selected for the interview.
