Background
The Social Security Administration (SSA) uses Form SSA-789 to request reconsideration of a decision to stop disability benefits (or to request an appeal for a disability hearing after an initial CDR cessation).
Revision to the Information Collection
Change #1: We are revising the Privacy Act Statements on this collection.
Justification #1: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statements on this collection.
We will implement the changes upon OMB approval. This action will not affect the public reporting burden.
